In a pilot randomized crossover study of 20 adults in New York City, the compact cuff-integrated SOMNOmedics ABPM Pro device produced markedly better participant experience than the traditional SpaceLabs 90227 oscillometric monitor across multiple domains. Comfort scores nearly doubled (median 7.0 vs 3.5, P=0.004), pain scores dropped by roughly 70% (median 1.5 vs 5.0, P=0.003), and embarrassment was significantly lower (median 0.5 vs 3.0, P=0.01) — while both devices met guideline-based recording-quality thresholds for valid 24-hour ambulatory blood pressure monitoring (ABPM).

Patient tolerability has long been the quiet obstacle undermining ABPM adoption, despite its recognized superiority over office measurements for diagnosing hypertension and predicting cardiovascular risk. Devices that cause pain or social embarrassment get removed prematurely or discourage repeat use, introducing measurement gaps that compromise clinical value. A more wearable form factor that preserves data quality could meaningfully expand access — particularly among working-age adults, shift workers, and those in community screening programs.

However, this preprint — posted on medRxiv and not yet peer-reviewed — carries important caveats. The 20-person pilot is underpowered for definitive conclusions, and the New York City sample limits generalizability. Crossover designs mitigate confounding but cannot fully eliminate order effects. Sleep interference and daytime activity ratings did not differ significantly between devices, moderating enthusiasm. Results should be considered hypothesis-generating until confirmed in larger, more diverse trials. An incremental but practically meaningful finding.
