Understanding exactly which pathogens are driving preventable deaths from contaminated food — and where — has major implications for everything from public health infrastructure to individual dietary risk awareness. A comprehensive new WHO-commissioned synthesis published in The Lancet Global Health now provides the most granular picture yet of foodborne diarrhoeal disease across two decades and 194 countries, and the numbers are sobering.

The analysis tracked 14 specific enteric hazards — including Campylobacter species, norovirus, rotavirus, Shigella, Salmonella, and multiple Escherichia coli pathotypes — estimating illnesses, deaths, and disability-adjusted life-years (DALYs) from 2000 through 2021. In 2021 alone, these pathogens collectively caused an estimated 666 million illnesses (95% UI: 483–884 million), approximately 265,000 deaths, and 15.2 million DALYs attributable specifically to foodborne transmission routes. The three leading contributors to disease burden measured in DALYs were Shigella, Campylobacter, and rotavirus. Geographically, sub-Saharan Africa bore a disproportionate share, with mortality rates 7.1 times higher than lower-burden regions, reflecting stark disparities in food safety infrastructure, sanitation, and healthcare access.

This update is analytically significant for several reasons. Prior WHO burden estimates from 2015 covered only a single reference year; this iteration models temporal trends across 22 years using hierarchical meta-regression with geographic clustering and rigorous uncertainty propagation — a methodological step forward. The inclusion of rotavirus is particularly notable given that rotavirus vaccination programs have expanded substantially since 2000, yet the pathogen remains among the top three burden contributors, suggesting vaccination rollout remains incomplete in high-burden regions. For Campylobacter, the data reinforce a long-standing gap between its outsized disease burden and the relative scarcity of targeted interventions compared to Salmonella. Limitations include the model's reliance on systematic reviews and expert judgment to fill data gaps in low-surveillance settings, which widens uncertainty intervals considerably. Still, this represents a confirmatory and incrementally paradigm-refining estimate rather than a surprise finding — the geography of food safety inequity is painfully consistent.
