For the millions of infants born small-for-gestational-age in low-resource settings, the first year of life represents a narrow but critical window where targeted support can meaningfully alter developmental trajectories. A well-powered randomized trial now provides some of the clearest human evidence yet that combining nutrition, health care, early stimulation, and psychosocial support into a single integrated package can shift growth and neurodevelopmental outcomes measurably within that window.
Conducted in low-resource neighborhoods of South Delhi, the trial enrolled 1,300 term SGA infants within 14 days of birth and randomized them 1:1 to either an integrated multidomain intervention or standard government-program care. At 12 months, 97% of participants remained in follow-up — an exceptional retention rate. Infants in the intervention arm averaged 8.0 kg compared to 7.8 kg in the usual-care group, a mean difference of 0.22 kg. The significance of this trial lies as much in its design as its findings. Multidomain interventions are notoriously difficult to execute and evaluate at scale — disentangling which component drives benefit is analytically complex, and real-world fidelity to protocols often degrades. That this study achieved near-complete follow-up in a low-resource urban Indian context strengthens its internal validity considerably. The 0.22 kg weight difference, while modest in absolute terms, may translate into meaningful reductions in stunting risk when sustained over time. What remains to be seen from the full publication is the magnitude of neurodevelopmental gains and whether subgroup effects — by sex, severity of SGA, or household poverty — reveal which infants benefit most.
