The 2026 SEPAR guidelines, developed through a systematic review using GRADE and ADOLOPMENT frameworks, recommend universal AATD screening at the time of COPD diagnosis, as well as testing in adults with late-onset asthma accompanied by persistent airflow obstruction and in patients with bronchiectasis of unknown origin. The cornerstone diagnostic threshold is set at serum AAT below 116 mg/dL by immunonephelometry, measured alongside a normal C-reactive protein level — an important qualifier since AAT is an acute-phase reactant that rises during inflammation, which could otherwise mask deficiency. When concentrations fall below this cutoff, a second confirmatory step characterizing either the genotype or protein phenotype is recommended.
