Despite decades of public health messaging linking ultraviolet radiation to melanoma and non-melanoma skin cancers, a substantial portion of the American adult population continues to seek sun exposure for cosmetic tanning purposes — a finding with direct implications for skin cancer prevention strategies and dermatological surveillance programs.

Drawing on the 2024 National Health Interview Survey — a nationally representative cross-sectional sample of 31,615 civilian US adults with a mean age of 48.3 years — this study quantified how often Americans intentionally expose their skin to sunlight to achieve a tanned appearance. Participants were asked directly about tanning-seeking behavior on a five-point frequency scale, as well as whether they had experienced any sunburn in the prior 12 months. The analysis also probed demographic correlates and health-related behavioral factors, including physical activity levels and alcohol consumption patterns, to identify which population subgroups carry the greatest UV risk burden from intentional tanning.

The epidemiological significance here extends beyond curiosity about sunbathing habits. UV radiation from solar exposure remains the dominant modifiable risk factor for all major forms of skin cancer, including cutaneous melanoma, squamous cell carcinoma, and basal cell carcinoma. What makes this study analytically useful is the granularity of its behavioral framing: intentional tanning is volitional, preventable, and distinct from incidental occupational or recreational sun exposure. Prior surveillance infrastructure tracked sunscreen use and sunburn incidence but left intentional tanning prevalence largely unmeasured at a national scale. The 2024 NHIS data fill that gap meaningfully. Key limitations include the cross-sectional design, which precludes causal inference, and the reliance on self-reported behavior — a well-documented source of social desirability bias in health surveys. Nevertheless, for a behavior this clearly linked to carcinogenic exposure, establishing population-level prevalence baselines is an essential step toward more targeted skin cancer prevention interventions.
