For families managing tuberous sclerosis complex, the neuropsychiatric burden has long been underappreciated relative to the disease's more visible physical manifestations. A large cross-sectional Chinese cohort now quantifies just how pervasive that burden is — and reveals that genotype shapes which neuropsychiatric features dominate, a detail with real implications for early clinical surveillance.
The study enrolled 311 consecutive TSC patients across two tertiary centers in China between March and December 2024, using a culturally adapted Chinese version of the validated TAND Checklist. Among those who received formal cognitive assessment, 68.4% met the threshold for intellectual disability (IQ below 70). Behavioral difficulties were nearly ubiquitous: mood dysregulation affected 67.8% of patients, language impairment 67.5%, and inattention 64.6%, with the average patient presenting eight distinct behavioral problems. Autism spectrum disorder emerged as the most common formal psychiatric diagnosis at 22.2%. Age-related patterns were also clear — younger children disproportionately struggled with language, self-care, and attention, while adults shifted toward anxiety and depression. Crucially, TSC1 and TSC2 genotypes showed statistically significant differences in TAND feature profiles, reinforcing the value of genotype-informed neuropsychiatric monitoring.
This dataset is meaningful in the context of a condition historically managed with primary attention to epilepsy and renal or pulmonary lesions. TAND has been formally recognized since 2012, yet screening gaps persist globally, and China's population-scale TSC burden has been poorly characterized. The absence of sex differences in TAND prevalence challenges assumptions borrowed from idiopathic autism research. The genotype-TAND link is particularly actionable — TSC2 mutations are generally associated with more severe disease, and if neuropsychiatric profiles diverge predictably by genotype, clinicians may be able to stratify monitoring intensity earlier. Limitations include the cross-sectional design, tertiary-center selection bias likely enriching for more severe cases, and incomplete IQ data for a significant subset. Nonetheless, this represents one of the largest TAND characterizations outside Western cohorts and adds meaningful geographic and genetic granularity to an underserved research area.
