For a condition that sits at the intersection of neurology and psychiatry, functional/dissociative seizures (FDS) remain dramatically under-researched in terms of lifestyle interventions. Millions of seizure-disorder patients carry an FDS diagnosis, yet clinical guidance on nutrition is virtually absent — making patient-driven forums the de facto information exchange. Understanding what these patients actually believe and practice nutritionally has real implications for closing that care gap.

This mixed-methods study mined the Reddit community r/PNESsupport, applying both sentiment analysis and reflexive thematic analysis to 31 comments from 23 FDS-affected users. The aggregate sentiment score was mildly positive (0.080 ± 0.116), suggesting cautious optimism rather than frustration. Five thematic clusters emerged: blood sugar stabilization and meal timing as perceived seizure triggers; ketogenic, low-carbohydrate, and elimination diets reported as symptom-modifying but flagged for hydration and nutritional adequacy risks; use of cannabidiol oil, amino acid supplements, and avoidance of artificial sweeteners as complementary strategies; food sensitivities and gastrointestinal discomfort linked bidirectionally to symptom burden; and nutrition framed as an adjunct — not a replacement — to medical and psychological care.

The study's central contribution is less its findings than its method: using social media discourse as a window into unmet clinical need. FDS, historically labeled psychogenic non-epileptic seizures (PNES), sits in a therapeutic no-man's-land where neurologists often defer to psychiatrists and vice versa, leaving patients to crowdsource dietary strategies with no evidence base to guide them. The gut-brain axis is a biologically plausible mechanism here — FDS patients commonly report gastrointestinal comorbidities and autonomic dysregulation — yet no randomized trials exist on dietary interventions in this population. The sample size of 23 users is strikingly small and drawn from a self-selected, English-language online community, limiting generalizability substantially. This is hypothesis-generating work at best, but it performs a meaningful service by mapping the patient-experience landscape and signaling where clinical investigation should follow.
