For adults 65 and older, every flu season carries disproportionate risk of hospitalization and death — so the question of whether a higher-antigen influenza vaccine meaningfully changes those odds matters enormously to public health planning and to individuals navigating preventive care options. This meta-analysis from The Lancet Healthy Longevity attempts to settle a debate that has persisted since high-dose formulations entered clinical use.

The systematic review pooled data from randomized and observational studies comparing high-dose inactivated influenza vaccine (HD-IIV, containing four times the standard antigen load) against standard-dose formulations in adults aged 65 and over, with endpoints including all-cause hospitalization, influenza-related hospitalization, pneumonia admission, and mortality. Across the pooled evidence, HD-IIV was associated with statistically significant reductions in influenza-related and pneumonia-related hospitalizations relative to standard-dose comparators. However, the absolute risk reductions were described as modest on average, and no consistent mortality benefit emerged across the full dataset.
