Declining childhood cancer mortality is one of oncology's genuine success stories, yet the pace of progress varies sharply by geography, socioeconomics, and healthcare access. Understanding where gains are occurring—and who is still being left behind—matters enormously for closing survival gaps in middle-income countries where pediatric oncology infrastructure remains uneven.
This ecological time-series study examined all leukemia-related deaths among individuals aged 0–19 in São Paulo State across 3,687 recorded fatalities between 2000 and 2019. Using Prais-Winsten regression on DATASUS mortality microdata, researchers identified a statistically significant downward trend with an annual percent change of −0.017, translating to a rate reduction from 1.69 per 100,000 in 2000 to 1.21 per 100,000 by 2019—roughly a 28% decline. Acute lymphoblastic leukemia (ICD-10 code C910) dominated, accounting for just over half of all deaths. Mortality burden skewed male (56%), predominantly white (71%), and notably elevated among the 15–19 age group (30.8%). Unspecified septicemia emerged as the most frequently coded associated cause of death, appearing in 25.8% of cases.
The downward trajectory broadly mirrors international trends driven by improvements in ALL treatment protocols—particularly the adoption of risk-stratified chemotherapy regimens and enhanced supportive care. However, several findings warrant scrutiny. The demographic skew toward white individuals likely reflects both São Paulo's baseline population composition and persistent healthcare access disparities rather than true biological risk differences. The high septicemia co-mortality signal is clinically meaningful: infection-related deaths during chemotherapy-induced immunosuppression remain a modifiable target for intervention through prophylaxis optimization and early-warning protocols. Critically, this is an ecological observational study—it cannot establish causation, and São Paulo's status as Brazil's wealthiest, best-resourced state limits generalizability to less-served regions. The study offers confirmatory epidemiological value rather than paradigm-shifting findings, but its systematic framing strengthens the evidence base for targeted public health resource allocation.
