Vascular dementia is frequently dismissed as the less-glamorous sibling of Alzheimer's disease, yet cerebral small vessel disease (SVD) underlies a substantial proportion of cognitive decline in aging populations. A comprehensive new review challenges researchers and clinicians to reckon seriously with inflammation — both systemic and central — as a potential driver of this largely undertreated condition, while being admirably candid about what remains unresolved.
The review synthesizes evidence from post-mortem neuropathology, in-vivo positron emission tomography using translocator protein (TSPO) radioligands, and large-scale blood proteomics. Microglial activation has been documented in SVD brain tissue and confirmed in living patients via TSPO-targeted PET imaging. On the peripheral side, proteomic panels have implicated proteins governing coagulation cascades, endothelial activation, and immune cell adhesion — alongside specific cytokines — as biomarkers associated with SVD burden. Coverage extends to sporadic SVD, cerebral amyloid angiopathy (CAA), and monogenic small vessel diseases such as CADASIL, offering a rare unified framework. Critically, the authors note that replication across cohorts is inconsistent and that prospective data linking individual inflammatory proteins to future disease progression remain sparse.
This review arrives at an important inflection point. The anti-amyloid antibody era in Alzheimer's research has intensified scrutiny of neuroinflammation broadly, and SVD — which lacks any approved disease-modifying therapy — stands to benefit from that momentum. However, the central limitation acknowledged here is the absence of proven causality: current evidence is largely associative, leaving open whether inflammation drives SVD progression or is simply a downstream consequence of vascular injury. Mendelian randomization and genetic approaches are cited as tools to interrogate directionality, though no definitive causal verdict is rendered. For clinicians and researchers, this is an incremental but methodologically important synthesis — a rigorous map of what is known and, perhaps more valuably, what experimental designs are needed before anti-inflammatory strategies can move confidently into SVD trials.
