Fatty liver disease has long lacked effective pharmacological options, leaving millions of patients with progressive fibrosis and few tools beyond lifestyle intervention. The August 2025 accelerated FDA approval of semaglutide 2.4 mg weekly for metabolic dysfunction-associated steatohepatitis (MASH) with moderate-to-advanced fibrosis marks a genuine inflection point — the first GLP-1 receptor agonist to receive an indication specifically targeting liver histology.

The approval rests on interim data from the phase 3 ESSENCE trial involving 72 weeks of subcutaneous semaglutide at 2.4 mg per week. Two primary histologic endpoints were met simultaneously: MASH resolution without fibrosis worsening occurred in 62.9% of treated patients versus 34.3% on placebo, and at least one stage of fibrosis regression without MASH worsening was achieved in 36.8% versus 22.4% on placebo — both highly statistically significant. The updated AASLD guidance formalizes patient selection criteria using validated non-invasive testing thresholds: vibration-controlled transient elastography between 8–15 kPa, magnetic resonance elastography between 3.1–4.4 kPa, or enhanced liver fibrosis scores of 9.2–10.5, deliberately steering clinicians away from routine liver biopsy.

Placing this in context, resmetirom (Rezdiffra) received traditional FDA approval for MASH in early 2024, making semaglutide the second entrant in what is now a nascent but rapidly evolving therapeutic field. Crucially, semaglutide's approval is accelerated rather than traditional, meaning final authorization awaits long-term cardiovascular and cirrhosis-progression outcomes from ESSENCE's complete dataset. This distinction matters: histologic improvement is a surrogate endpoint, and whether fibrosis regression translates into reduced liver-related mortality remains unconfirmed. Additionally, semaglutide's concurrent metabolic benefits — weight reduction, glycemic control, cardiovascular risk reduction — complicate attribution of liver-specific effects and simultaneously broaden its clinical appeal for patients with overlapping metabolic comorbidities. The guidance represents an important clinical infrastructure update, though the evidence base is still maturing.
