As GLP-1 receptor agonists become among the most prescribed medications globally — used not just for diabetes and obesity but increasingly for cardiovascular and metabolic conditions — their side-effect profile is under intensifying scrutiny. Hair loss has emerged as a patient-reported concern, yet until now the evidence has been fragmented across individual trials. This meta-analysis provides the first pooled quantitative estimate of that risk, and the numbers are harder to dismiss than anecdotal reports.
Drawing from nine interventional studies — seven of which were randomized controlled trials — and analyzing outcomes from 4,114 GLP-1 RA users, the pooled risk ratio for hair loss versus placebo reached 3.25 (95% CI: 1.44–7.36). When restricted to RCTs enrolling patients specifically with overweight or obesity, the effect size strengthened to a risk ratio of 3.59 (95% CI: 2.10–6.12). In absolute terms, the single-arm analysis estimated that roughly 1 in 26 GLP-1 RA users — an event rate of 3.9% — will experience hair loss during treatment.
Several mechanisms deserve consideration, though none are yet confirmed. Rapid caloric restriction and accelerated weight loss are established drivers of telogen effluvium, a stress-induced shedding of resting-phase hair follicles. Since GLP-1 agonists powerfully suppress appetite, the hair loss observed may be partly a metabolic consequence of rapid weight change rather than a direct pharmacological effect on hair follicle cycling. Disentangling drug-specific from weight-loss-mediated effects will require trials that carefully control for rate and magnitude of weight reduction. Additionally, the heterogeneity across included studies — different agents, dosing regimens, and follow-up durations — means the pooled estimate carries meaningful uncertainty, reflected in the wide confidence intervals. With only nine studies and a combined sample of just over 4,000 users, this meta-analysis is best interpreted as confirmatory signal rather than definitive causal evidence. Nonetheless, for clinicians and patients weighing GLP-1 therapy, hair loss now warrants explicit pre-treatment counseling.
