For adults who repeatedly lose and regain weight while ending up at roughly the same number on the scale, the internal damage may be invisible yet measurable — and it disproportionately affects muscle rather than fat. This distinction matters enormously for long-term mobility, metabolic health, and osteoarthritis risk, especially in middle-aged and older populations.
This secondary analysis of the Osteoarthritis Initiative — a well-characterized longitudinal cohort — isolated 1,433 participants whose body weight remained within 5% of baseline over 48 months, then separated them into weight cyclers (n = 77) versus stable noncyclers (n = 1,356) using MRI-derived measurements of the right thigh. Weight cyclers experienced nearly 3.7% muscle volume loss over four years compared to roughly 1% in noncyclers — a statistically significant difference (P = .003) after adjustment for age, sex, metabolic, and lifestyle confounders. Notably, intermuscular adipose tissue (IMAT) infiltration increased at virtually identical rates in both groups, and no significant difference emerged in knee-adjacent subcutaneous fat thickness, suggesting the harm from cycling is selectively muscular rather than broadly obesogenic.
This finding adds meaningful granularity to an already contentious debate about weight cycling. Prior research has raised concerns about cardiovascular and metabolic consequences of yo-yo dieting, but direct MRI evidence isolating skeletal muscle loss — net of overall weight change — is relatively novel. The selective muscle atrophy pattern aligns mechanistically with what exercise physiologists call "anabolic resistance": each cycle of caloric restriction may progressively impair the muscle's capacity to rebuild during weight regain phases, a phenomenon better documented in older adults. The cohort's mean age of ~61 years makes this particularly relevant for osteoarthritis and fall-risk trajectories. Key limitations include the modest cyclist group size (n = 77), the observational design precluding causality, and the possibility that cycling intensity or dietary composition — unmeasured here — confound the relationship. Nonetheless, for a health-conscious population prioritizing body composition over scale weight, this is incrementally important evidence.
