Tuberculosis remains one of the most consequential infectious diseases in human history, and for the first time in decades, the world has a granular 33-year accounting of how the epidemic has evolved — and where it is heading. That accounting now matters urgently, as global health funding cuts threaten to unravel progress at precisely the moment drug resistance is accelerating.

The Global Burden of Disease Study 2023 analyzed TB mortality and morbidity across 204 countries from 1990 to 2023, integrating vital registration data, surveillance systems, verbal autopsies, and minimally invasive tissue sampling into ensemble cause-of-death models. TB deaths declined substantially over the study period, with HIV-related TB mortality particularly responsive to antiretroviral therapy scale-up. However, multidrug-resistant tuberculosis (MDR-TB) has emerged as a counterweight to those gains, with its burden quantified separately using population attributable fraction methods that stratify cases by HIV co-infection and drug-resistance status. Disability-adjusted life-years — combining years of life lost with years lived with disability — were also computed, providing a fuller picture of TB's total health impact beyond mortality alone. Alcohol use, smoking, and elevated fasting plasma glucose were identified as quantifiable risk factors driving a measurable portion of the TB burden through a comparative risk assessment framework.

This study's scope and methodology place it among the most comprehensive TB burden assessments ever published, yet several limitations deserve attention. GBD estimates rely heavily on modeled data in regions with weak vital registration systems — precisely the areas with the highest TB burden — introducing uncertainty intervals that can span wide ranges. The 2023 cutoff predates any measurable effect of recent funding disruptions to programs like the Global Fund, meaning the headline progress figures may represent a historical ceiling rather than a sustained trajectory. MDR-TB trends are particularly concerning: treatment success rates remain low globally, second-line drug pipelines are thin, and resistance surveillance is incomplete in high-burden nations. For health-conscious adults in low-prevalence settings, the key implication is indirect — TB's resurgence, if it occurs, would signal broader infectious disease system failure with cross-cutting consequences for global health security.
