Telomere biology has long sat at the center of longevity research, but a growing body of evidence suggests the enzyme responsible for maintaining those telomeres — telomerase reverse transcriptase, or TERT — does far more than protect chromosome ends. Understanding TERT's full functional portfolio could fundamentally reshape how researchers design and evaluate anti-aging interventions targeting the telomere system.
This Perspective in Nature Aging synthesizes current evidence around TERT's dual identity: its canonical role in extending telomeres by adding repetitive nucleotide sequences to chromosomal ends, thereby countering replicative senescence, and its noncanonical functions that extend into gene regulation, mitochondrial protection, DNA damage response, and inflammatory signaling. The authors construct an evidence-based framework specifically designed to evaluate geroprotective strategies — interventions aimed at slowing or reversing aging — that target either TERT directly or the broader telomere-maintenance machinery. The framework distinguishes between approaches that amplify TERT's protective signaling versus those that risk unintended oncogenic consequences, a long-standing concern in the field.

As a synthesis rather than a primary clinical trial, this work is inherently hypothesis-organizing rather than hypothesis-testing, but its value lies in consolidating a fragmented literature into actionable research priorities.
