Idiopathic pulmonary fibrosis has long resisted meaningful therapeutic progress — current antifibrotic drugs slow decline but rarely halt it, and median survival after diagnosis remains under five years. A large, well-designed randomized trial now offers evidence that a repurposed inhaled therapy may meaningfully change that trajectory, including for patients already on standard-of-care treatment.

The phase 3, double-blind TETON trial enrolled 593 patients with IPF, randomizing them to inhaled treprostinil (12 breaths, four times daily) or placebo over 52 weeks. Treprostinil, a prostacyclin analogue previously approved for pulmonary arterial hypertension, appears to exert antifibrotic effects through distinct pathways beyond vasodilation. The primary endpoint — change in absolute forced vital capacity (FVC) at week 52 — showed a median decline of approximately 49.9 mL in the treprostinil group versus 136.4 mL in the placebo group, representing a roughly 63% relative reduction in lung function loss. Critically, about 75% of participants were concurrently receiving background antifibrotic therapy (pirfenidone or nintedanib), making the add-on effect particularly clinically relevant. Secondary endpoints including clinical worsening and acute exacerbation events were also assessed in a prespecified hierarchical analysis.

This result is potentially paradigm-shifting for IPF management. The disease's progressive, irreversible fibrosis has made FVC preservation the gold-standard surrogate endpoint, and effect sizes of this magnitude on top of existing antifibrotics have not been demonstrated before. That said, important caveats apply: the trial population skewed heavily male (80%) and elderly (mean age 71.7), limiting generalizability; the inhaled route requires significant adherence (48 breaths daily); and long-term safety data beyond 52 weeks remain unavailable. Whether the FVC benefit translates to improved survival or hospitalization rates at meaningful scale will require post-approval surveillance. Still, for a disease with few options, this represents a genuinely significant advance.
